COHMIS Paper Intake Forms
The following video demonstration explains how to complete a paper intake form when entering data directly in HMIS is not an option.
Paper forms serve as a resource to help guide data collection in alignment with Fiscal Year 2026 HMIS Data Standards. The video below highlights best practices for completing the forms and helps to provide context and clarify nuances associated with the data collection requirements of our system.
User Access and Permissions
Providers with full COHMIS access may enter data directly into COHMIS without completing paper forms, these client forms are for providers who do not have full COHMIS access, or providers who prefer using paper forms and later transposing data into the system.
