This article includes a list of the various reports offered in the Clarity Report Library, along with brief descriptions for each report and links to the corresponding Clarity Help Center article where you will find further details about the report. You can also find these articles in the Report Library section of the Clarity Help Center.
If you do not see a report in the Report Library that meets your needs, you may review the Looker Resources article which discusses the advanced reporting tool offered by Clarity Human Services to create custom reports, which is available to users with Looker licenses. You may also submit a ticket to your COHMIS Lead Agency to inquire about obtaining Looker access and/or request a custom report.
Agency-Specific Reports

[CO-101] Swipe Card Batch Report: This report enables you to quickly print multiple scan cards for clients. This is particularly useful in shelter situations, where you may have a large number of clients who need scan cards all at once. (Because this report was specifically requested by COHMIS, this report is not mentioned on the Clarity Help Center and therefore there is not a link included here.)
Agency Management Reports

[STFF-101] User Activity Report - Generates data reflecting the system activity of certain staff members. Pulling from the System Dashboard, this report provides information regarding login dates, times, and durations.

[STFF-103] User Active Caseload - Provides details of the current caseload for a user and the activities. A staff member is considered assigned to an enrollment if they created the enrollment or were assigned to the enrollment within Clarity Human Services.

[STFF-104] Staff Client Data Activity - Provides a record of each staff member and their activities in the system, such as creating/editing a client profile, creating a program enrollment, or providing a service.

[STFF-106] User Client Note Hours Tracked - Provides program management with an overview of the client note hours that were tracked by users. The hours also include a drilldown to details about the clients and notes tracked.
Assessment-Based Reports

[GNRL-210] Assessment Details Report - Lists all the responses to the questions on the selected assessment for each client. Clients must have an assessment during the reporting period to be included in the report, but do not need to have an enrollment.

[GNRL-404] CE Assessment Details Report - Shows a list of all clients assessed under the selected assessment(s) during the report period and the corresponding assessment score details. This report is similar to the [GNRL-401] VI-SPDAT Details report, except that it expands the assessment choices to all assessments designated for coordinated entry, not just the VI-SPDAT. This version also only reports subtotals of the assessment processors, not the details about each assessment field.

[GNRL-405] CE Assessing Staff Report - Lists assessments created during the reporting period, separated by the assessing staff person. It is intended for use by assessing staff and program management. For assessments to appear in this report, they must have Coordinated Entry toggled on and have at least one linked Assessment Processor.

[OUTS-108] CE Demographics Report - Provides a way for communities to understand the demographics of the people served by the coordinated entry system.
Client Reports
These reports are client-based and must be run from the client's profile screen.

[CLNT-101] Case Notes - Provides a method for case managers to run printed case notes (either notes linked to project enrollments or standalone notes) for inclusion in case files. Be careful to secure the printed case notes to protect client privacy.

[CLNT-102] Client History - Provides a history of services provided, program enrollments, and Inventory related assignments for the client for the selected reporting period.

[CLNT-103] Photo ID Card - Sample - Provides a simple method for printing ID cards which can be used with any barcode scanner for providing services within Clarity Human Services.

[CLNT-104] Profile Screen - Lists the current basic demographic data on the Profile Screen.

[CLNT-105] Client Appointments - Provides a list of appointments scheduled for the client during the reporting period.

[CLNT-106] Client Service Notes - Provides a list of service notes written during the reporting period.

[CLNT-125] Client Summary - Provides an overview of the client's history in the system. This report pulls data from many of the client-level "interactions" with Clarity Human Services.

[CLNT-127] Homeless Status Timeline Report - Provides a visual representation of a client's timeline across HMIS enrollments. It indicates the months in which a client’s homelessness or housed status is documented through HMIS enrollment data and months in which homelessness cannot be determined through HMIS enrollment data.

[CLNT-128] Client Enrollment Details - Provides information on client responses to questions about their history of homelessness as well as move-in and destination at exit information. These details can help determine additional periods of homelessness for clients.
Community and Referrals

[GNRL-401] VI-SPDAT Details - Provides a list of clients assessed during the reporting period and VI-SPDAT details, including section scores, age, gender, veteran status, and if a referral was made or contact information added.

[RFRL-102] Referral Outcome Statistics - Provides a detailed breakdown of referral outcomes by agency and program during the reporting period. An Administrator version of the report is also available: [RFRL-102-AD]. System Administrators and users with additional access can run the report.

[RFRL-103] Referral Statistics - Inbound - Provides aggregate counts of referrals, broken out by different statuses. In contrast to the Outbound report, this version reports on referrals that were referred to the agency running the report.

[RFRL-104] Referral Statistics - Outbound - Provides aggregate counts of referrals, broken out by different statuses. In contrast to the Inbound report, this version reports on referrals that were created by the agency running the report.

[RFRL-110] VI-SPDAT Referral Detail - Based on referrals that were created during the Report Date Range. The report is broken down by referral types (Community Queue, Agency Referrals).

[RFRL-120] Community Queue Detail - Provides details about clients that were referred to the Community Queue by the agencies with the same CoC as the current agency.

[RFRL-122] CE Community Queue Detail Report - Provides details about clients on coordinated entry queues.

[UNIT-102] Unit Client Detail - Referral - Provides details on referred clients who were assigned to a Unit during the Report Date Range.
Data Quality Reports

[DQXX-102] Program Data Review - Provides a list of client enrollments, key enrollment details (e.g., entry date, exit date, days active in program), and highlights missing program entry and program exit data.

[DQXX-103] Monthly Staff Report - Provides three categories of information: (1) General data quality; (2) User Activity, including the number of clients that each staff member worked with during the time frame of report; and (3) Data quality by data element (e.g., Date of Birth, Race and Ethnicity, Veteran Status) for all clients served. The report also includes staff from other agencies with "additional access" to the reporting agency data.

[DQXX-110] Duplicate Clients - Helps identify duplicate client profiles in the system by providing a list of client records that have identical Personal IDs. (Personal IDs are automatically generated by the system when personal identifying information in one client record matches that of another client record.)

[DQXX-121] Project Start Date > Project Exit Date - Provides a proactive view of project enrollments where the Project Exit Date is before the Project Start Date. These situations can occur during data integration and in some situations where the project start or exit date are updated after the fact.

[DQXX-122] Duplicate Assessments Report - Identifies any clients who have more than one of the same type of assessment completed within the date range.

[GNRL-242] Client Project Stay Issues - Primarily designed to identify problems created during Data Integration. These problems are likely to occur if more than one person is performing the data integration for a community. These problems may result in trouble generating other reports such as APR and ESG-CAPER.
Housing Reports

[HSNG-102] CoC Housing Assessment Report - Uses Bed Inventories and Program Enrollments to report on bed availability and utilization. Provides a monthly housing attendance average for each month of the reporting period for all CoC-participating agencies. The four PIT (Point-in-Time) dates (last Wednesday of the designated month) are also provided to assist in the assessment of individual housing programs included in the HIC (Housing Inventory Count) and the former AHAR (Annual Homeless Assessment Report), now derived from the LSA (Longitudinal System Analysis).

[HSNG-104] Monthly Housing Report - Provides a complete single-month review of housing programs. This report breaks down lodging and residential project stays into sections by project type. Each table includes the program name, the average number of occupied beds and total available beds, data quality percentage, and count of unique clients.

[HSNG-105] Weekly Housing Census - Provides a seven-day review of housing programs. This report breaks down lodging and residential project stays into sections by project type. Each table includes program inventory (beds/units), attendance and data quality by date, and information on household size.

[HSNG-108] Housing Census - Provides a day-by-day account of the bed occupancy during the reporting period. This is broken out into separate tables by programs selected. This report includes all residential programs (emergency shelter, safe haven, transitional housing, and permanent housing programs).

[HSNG-200] Current Housing Availability - Summarizes (by housing project type, then program name) the amount of available bed and unit inventory on the night before the report is run.
HUD & Federal Partner Reports

[HUDX-111] HUD CSV / XML Program Data Export - This report is used for exporting data from the Clarity system according to HMIS CSV/XML Format Specifications.

[HUDX-123-AD] Housing Inventory (HIC) Supplemental - Tool to review housing inventory available for people experiencing homelessness. The report offers a point-in-time look at utilization while also being used as a visual tool to accompany LSA review for HIC reporting submissions. Logic in this report heavily relies on mimicking the LSA where relevant, while also including additional data points to aid in reporting review and analysis.

[HUDX-230-AD] Shelter Count PIT - Designed to report the count of clients sheltered during a chosen point-in-time. The report is required during the once-per-year HIC/PIT reporting, usually in the Spring.

[HUDX-223-AD] System Performance Measures - HUD report designed to report on a CoC's progress in meeting the needs of people experiencing homelessness. This report analyzes data from continuum projects for a CoC to review system-level performance. These measures are used as competitive elements in annual CoC Program Competitions.

[HUDX-224] PATH Annual Report - Program and service based report that provides details of all clients according to PATH Reporting Specifications.

[HUDX-225] HMIS Data Quality Report - Reviews data quality across a number of HMIS data elements, including data elements found on the client profile (e.g., Name, Social Security Number, demographic details), data elements within a program enrollment (e.g., Project Start Date, Disabling Condition, Income and Sources, Non-Cash Benefits, Destination), data elements used to determine chronic homelessness (e.g., Prior Living Situation questions), data entry timeliness, and inactive records.

[HUDX-227] Annual Performance Report (APR) - Report that recipients of HUD Continuum of Care (CoC) funding are required to submit to HUD every operating year. The report provides a series of tables with questions about service levels and outcomes for the funded projects. This report also reviews data quality across most of the same HMIS data elements as the HMIS Data Quality Report.

[EMPL-101] Employment Report - Lists the client employment history for clients who are enrolled and/or exited from a program during the reporting period AND received an employment service during the same period. This report pulls data only from the employment services connected to the selected program enrollment. It does not reflect employment statuses in the client profile screen or program enrollment screens.

[EMPL-102] Employment / Education Report - Lists the employment and education status for clients who are currently enrolled and/or exited from the selected program during the selected reporting dates.

[EXPS-103] Program Funding Source Financial Detail - Provides a detailed list of expense records for program-connected services.

[GNRL-105] Program Participation Summary - Provides basic program information for both active and inactive clients (with the option to include household members) for the specified program(s) during the report dates.

[GNRL-106] Program Roster - Provides a list of clients who match the selected status (e.g., active, enrolled, or exited) within the report date range for the selected program(s). Household members are grouped together in the list. This report includes client profile screen information (e.g., name, unique identifier, birth date, current age) and program stay information (e.g., age at entry, enroll date, exit date, length of stay, housing move-in date, count of assessments, count of services, count of case notes, assigned staff). The report also includes relevant inventory information for communities utilizing the Inventory functionality in Clarity Human Services.

[GNRL-220] Program Details Report - Lists all the fields and corresponding responses for the selected screen(s) (e.g., Entry, Status, Annual, Current Living, Exit, Follow-Up) associated with the selected program(s). Additionally, the report provides some profile and Housing Service information, depending on the screen selected.

[GNRL-400] Program Linked Service Review - Provides a detailed list of all services linked to the selected program(s). Clients are separated by a color block and the services are listed by date provided. Expense amounts, both separate and accumulative per client, are included.

[GNRL-406] Service Census [Program Based] - Provides a count of clients that received selected services per day within the report date range. Unlike the [GNRL-103] Service Census report, this report has a table for each program to differentiate where the client received the service. Drilldown functionality generates all client names for those who received each service, as well as a data quality score, and the staff member responsible for entering the information.

[GNRL-407] Service Summary [Program Based] - Similar to the [GNRL-104] Service Summary report, this report provides a number of unique clients, number of services, and number of service days per selected service within the report date range. [JRSD-104] Zip Code Jurisdictional Breakout - Lists the number of individual clients and families in a program by zip code. For the same report designed for services, use [JRSD-103] Zip Code Jurisdictional Breakout.

[OUTS-101] Program Outcome Measures - Provides program outcome information that includes program client exit information, housing status of exited clients, and efficiency/process measures. It also provides agency participation totals, and unduplicated counts of clients exited from either each program category and/or collectively.

[OUTS-102] Performance Monitoring - Measures program performance and acts as a performance evaluation tool to indicate program achievement in specific performance standards. It is intended to be used for the NoFA competition and can be used in comparing program performance.

[OUTS-105] Client Demographics [Service Based] - Provides client demographic details for selected services, for a specified time period, in a series of charts. For the report that is designed to collect demographic for programs, use [OUTS-106] Client Demographics. For a version of a service based demographic report that does not include charts, graphs, and figures, use [OUTS-105-F] Client Demographics (Flat Version). The information for this report comes from the client profile screen.

[OUTS-105-F] Client Demographics (Flat Version) - Provides client demographic details for selected services for a specified time period. This report provides the same information as [OUTS-105] Client Demographics, but charts, graphs, and figures are excluded from this report. As with [OUTS-105] Client Demographics, the information for this report comes from the client profile screen.
