This is the page we will use to host resources related to our upcoming 2025 Point-in-Time (PIT) count for the Balance of State (BoS) Continuum of Care (CoC), scheduled for the evening of Tuesday, January 28, 2025. Resources will continue being added to this page as they are developed.

Electronic Surveys
Please note: These electronic surveys use the Survey123 application, which can be accessed via web browser or a downloaded mobile app. To use Survey123 on the web browser, you must have reliable internet connection, while the Survey123 mobile app can store survey data offline (and then you can submit the surveys when you're back online). For information on how to use these electronic surveys, please see this short Survey123 video tutorial. We recommend downloading the Survey123 app on your mobile device (and familiarizing yourself with it) ahead of time in case of unreliable internet connection.

Paper Surveys
Please note: The BoS team will only be able to use electronic survey submissions (see links above) for the 2025 PIT count, including the Youth Supplemental Survey (YSS). If you complete paper surveys, someone within your region or organization will need to enter them electronically. If you have any questions or concerns about this, please reach out to the BoS Team at dola_bos_coc@state.co.us.
PIT paper surveys:
YSS paper forms: For the YSS paper forms, there are two pages. What you use for Page 1 depends on where the youth is residing. Please pay close attention to the instructions below for how to use these paper forms.
- YSS Standalone Form - Use this standalone form as Page 1 if the youth is experiencing homelessness or housing stability in any situation that is not in an Emergency Shelter, Transitional Housing, or Safe Haven. However, if the youth is in an Emergency Shelter, Transitional Housing, or Safe Haven, use the standard PIT Count survey above as Page 1 instead.
- YSS, Page 2 - Use this form as Page 2 for all YSS surveys.
HMIS Tutorial Videos
Creating Current Living Situation Assessments for Unsheltered Count - While most staff/volunteers will use PIT surveys for the unsheltered count, there is also the option of creating a Current Living Situation assessment for someone who already has a Street Outreach or Coordinated Entry program enrollment in the Homeless Management Information System (HMIS). The Current Living Situation assessment simply documents that the person was in an unsheltered location on the night of the PIT Count.
