Please find updated HMIS Client Forms below for the Balance of State Continuum of Care. We have updated these forms in accordance with the FY2024 HMIS Data Standards, which went into effect on October 1, 2023.
We have recorded the following tutorial video, which walks you through the best practices associated with completing the Balance of State client forms. We strongly recommend watching this video to anyone who will use these forms to collect information from clients, regardless of whether they use HMIS or not. Watching this video will help ensure they understand the context and nuance associated with HMIS and the data collection requirements of our system.

The forms below are to be used to collect data for the Youth Homelessness Demonstration Program (YHDP). These forms have YHDP-specific data collection requirements:

Region-Specific Coordinated Entry Forms
As part of ongoing efforts to refine Coordinated Entry processes in the Balance of State, certain regions have begun implementing additional questions to support care coordination and prioritization needs. Region-specific forms are available below:
- Southwest Colorado Supplemental Coordinated Entry Questions - Providers in the Southwest Colorado region have identified Supplemental Questions to help provide a clearer picture of a household's housing history, chronic homelessness status, health vulnerabilities, and support systems. These questions only need to be completed for the Head of Household.
